(If you save a copy of the on line form to your desktop the email button will not function in that the result will not be readable when it reaches us. However, you may fill it out on your computer then attach it to an email to operations@deepvac.com or simply fax it to us at 1-415-727-4200 – remember you must use the 1-415 even from within the 415 area code for this is an eFax number)

Definitions of Services and Instructions for Filling Out a Service Request Form
INSTRUCTIONS FOR FILLING OUT SERVICE REQUEST FORM
Note: Tenants are required to provide a 30 day notice of their intent to vacate which allows you the ability to schedule service requests up to a month in advance.
- Use only black or dark blue ink.
- Should there be any changes to the dates you need services they can easily be made by submitting another Service Request Form with the words “CHANGE ORDER” written across the top. The minimum time to request services is three business days notice and as always we will do our best to accommodate you in any unforeseen circumstance.
- Include the date and time you want the work done (please DO NOT write ASAP),
- If there is no rush say: “No Rush”,
- If it must be done by a specific date and time say: “Deadline”
- Include the building number and the apartment number to be serviced.
- Include the number of bedrooms, number of baths and “TH” for townhouse. For example:
- 1x1
- 3x2 TH
- Include the kind of service
- H = housekeeping /F for full, /P for partial or /S for spiff.
For example: H/F for Full Housekeeping.
Housekeeping services cover cleaning of all surfaces, dusting blinds and vacuuming the unit. The level of cleaning required is to be designated by Spiff, Partial, or Full as explained below. This is a general guideline:
Full: The unit is dirty from top to bottom and needs to be scrubbed to remove dirt and grease from all surfaces and appliances. This assumes that the resident did maintain good housekeeping practices throughout his/her tenancy and had elected not to clean anything upon move out
Approx Time frame is 1.5 – 2 more hrs than a partial. - C = Carpet Cleaning (CR = Carpet Repairs and explain what needs repaired in Notes section at bottom of page)
- P = Painting /F for Full, /P for partial or /TU for Touch-Up.
For example: P/F for full paint.
(Speak with Daren Holiday regarding definitions of Full, Partial and Spiff painting at your property.) - FC or FV= Flooring Installation of Carpet or Vinyl
Partial: The resident had made some attempts at cleaning but has not done very well. Most items need to be cleaned or re-cleaned. Approx Time Frame: 1 br = 3.5 hrs, 2 br = 4.5 hrs and 3 br = 5.5hrs.
Spiff: The unit has been cleaned once, by the tenant or someone else. The housekeeper just needs to wipe down everything in the bathroom and kitchen, dust the blinds and window sills and vacuum the carpet. This type of cleaning is usually required when a resident has done a great job cleaning the unit but has missed some fine details. Appliance or tub/shower cleaning would not be in this category. Approx Time Frame: 1br = 2 hrs, 2br = 3hrs and 3br = 3.5 hrs
Extra work: Units that are extremely dirty or damaged by cigarette smoke residue will incur additional charges at the rate of $30 per hour
